Referral Flow Chart
STAGE 2
The placement team reviews referral and decision if a possible match or not.
STAGE 3
If a possible placement is identified, this will be passed onto the Home Manager.
STAGE 4
If the social care team and residential team agree on suitability, the home management team will:
Meet with Social Worker for a matching meeting
Visit the child to make an informal assessment
Placement Plan Meeting
STAGE 5
After informal assessment and the Home Manager still agrees with the suitability of the child, then they will:
Create an impact risk assessment
Placement plan finalised
Child transition begins (1-week process unless more time is needed). This can include an initial visit to the home, and the child is happy. Coming to the home for lunch/ dinner or both and an overnight stay had.

If the visit raises any concerns for the child or the residential team, then the move may be stopped, and this will be referred to the placement team.
