Referral Flow Chart

The placement team reviews referral and decision if a possible match or not.

STAGE 3



If a possible placement is identified, this will be passed onto the Home Manager.

STAGE 4

If the social care team and residential team agree on suitability, the home management team will:

  • Meet with Social Worker for a matching meeting

  • Visit the child to make an informal assessment

  • Placement Plan Meeting

STAGE 5

After informal assessment and the Home Manager still agrees with the suitability of the child, then they will:

  • Create an impact risk assessment

  • Placement plan finalised

  • Child transition begins (1-week process unless more time is needed). This can include an initial visit to the home, and the child is happy. Coming to the home for lunch/ dinner or both and an overnight stay had.

  • ​

If the visit raises any concerns for the child or the residential team, then the move may be stopped, and this will be referred to the placement team.
